Does your webhost support PHP?

If the PHP code is displayed then it either means that the page extension is not PHP or the server does not support PHP.

This indicates an error in the code.
Why are you pasting PHP code? There is no need to write any code yourself for basic forms, you can use the built-in form processor.

It think the reason for this is because you have associated a link to the main item.
So instead of displaying the sub menu the browser will follow the link.

Tip: never use links in the main item when it has sub menus. There is no 'hover' on mobile devices.

Note that mobile device do not support 'hover' effects because there is no mouse.
I recommend to use a different type of menu for the mobile view.

I don't think 'browser style:true' will affect the behavior of uploading. The style is not related to the functionality (which is implement in the browser). I think you will have to add 3 different 'file upload' object to the form.
Make sure they all have a unique name.

Do you means you want to insert photos directly from the 'Photos' app of the Mac?
This is not possible, you will need to select an actual file on your harddisk, like an jpg or png.
